Leadership Weekend Attending Form

Please confirm with the form below your attendance at the Leadership Weekend on 22nd & 23rd September 2012. Work your way down the form completing the boxes. Those with a star next to them are compulsary.

Medical information on this form will compliment anything already registered on your Generic Health Forms. If we do not hold a generic health form for you - you will be required to complete one on arrival at the event.

This event is free of charge - including the evening meal. Participants MUST be 18 years or over. The event is open to both Scouting and Non Scouting members. On receipt of your form an information pack will be sent by return mail.
